2. If you're not satisfied, complete the online money back offer form online and print. No handwritten forms will be accepted.
3. Write the reason for your return and the 12 digit UPC bar code from the package on this form.
4. Include original or legible photocopy of your sales receipt. Sales receipts must clearly show store name, store address and purchase date.
